Texas Urology, we strive to make your experience at our clinic as
comfortable and convenient as possible. Therefore, we are providing these
forms for you to complete in the comfort of your own home. These are forms
which must be filled out prior to seeing the doctor for the first time.
If you have been to the clinic before you do not have to worry about
filling out these forms as you have already completed them.
To download the forms simply click on the name of the form you wish to
download. All forms can be read in Adobe Acrobat. If you don't have it, you
can get it here (for free).
Once you download the form you can then print it to fill out and bring with
you when you come to the doctor.
